What health cover does a KITAS holder actually need in Indonesia?

Three things stack, and confusing them is what costs people money. BPJS Kesehatan is the national scheme; where you are enrolled through an employer it is a legal matter, not a choice. Your employer’s plan, when there is one, usually caps well below what a foreigner-grade hospital charges. International cover is what fills the gap.

The mistake we see most often: buying a travel medical plan and treating it as annual expat cover. Genki Traveler is a travel plan of 1 to 12 months, with a €1,000,000 limit and no routine, dental or optical care. It is a legitimate stop-gap between two situations — it is not annual residency cover.

For annual cover as a resident the routes are IMG Global Medical Insurance, which is medically underwritten and accepts applications to 74, or an international insurer — Cigna, AXA, Allianz, Bupa — on quote. None of them publishes a premium we can print, because it depends on your age, your history and the tier you choose.

THE STARTING POINT

KITAS types, and what each one changes for your cover

The permit itself does not buy you health cover. What it changes is who is responsible for arranging it, and whether an employer sits in the chain.

Work KITAS (employer-sponsored)

An Indonesian employer sponsors the permit. In practice this is where an employer health plan appears — and where the question becomes what that plan actually covers at a foreigner-grade hospital.

Investor KITAS

No employer in the chain. You arrange everything yourself, which usually means an international plan on quote, plus BPJS where it applies to you.

Retirement KITAS

Age changes the picture more than the permit does. Entry ages and premiums after 60 are set individually — see the seniors guide for the age brackets.

Remote-worker and family permits

Dependants each need their own cover. A family tier on an international plan is priced individually, on quote.

Whatever the permit, the question to answer first is not « which plan is best » but « what does an admission at BIMC or Siloam cost me, and who pays it ». The rest follows from that.

THE NATIONAL SCHEME

BPJS Kesehatan: what the rule says, and what we will not overstate

BPJS Kesehatan is Indonesia’s national health scheme. Where you are a legally employed worker, enrolment is arranged by your employer. Beyond that, the rule and its enforcement have moved over the years, and they are applied differently from one office to another — so we state it the way our health insurance pillar states it, and no further.

We do not print a firm deadline after which BPJS becomes mandatory for every foreigner. Pages that do are stating a regulation more confidently than the practice supports. Confirm your own situation with your employer or with BPJS Kesehatan directly.

What BPJS gives you in practice

  • Care is delivered through an assigned first-tier facility (Faskes 1), with referral upwards from there — not direct access to a private international hospital.
  • Room class is chosen at enrolment and determines the ward you are admitted to.
  • Contribution levels are set by regulation and change; we do not print them here, because a figure that shifts is worse than none.
  • For an expatriate used to BIMC or Siloam international departments, BPJS is a floor, not a substitute.

Employer plans: the six questions worth asking

Most KITAS holders discover their employer plan’s limits at the admissions desk. These six questions take one email and settle it in advance.

What is the annual medical limit, and is it per person or per family?

This is the number that decides whether a foreigner-grade admission is covered or partly covered. Ask for the figure in writing.

Which hospitals are in the network, and is BIMC or Siloam among them?

A plan that does not deal with the hospital you would actually use is a plan you will pay in advance and claim back — if at all.

Is there direct billing, or do I pay and claim?

Direct billing is the difference between showing a card and finding a deposit. Ask which hospitals accept it.

Is medical evacuation included, and to where?

Serious cases in Indonesia are transferred, often to Singapore. A plan without an evacuation benefit leaves that transfer to you.

Are pre-existing conditions covered, excluded or loaded?

Employer schemes vary widely. Get the answer before you need it, not after a declaration form.

What happens to the cover the day I leave the job?

Most employer cover ends with the contract. Knowing the notice you get is what lets you arrange the next plan without a gap.

Why a travel medical plan is not annual expat cover

This page previously said otherwise, and it was wrong. The distinction is worth stating plainly, because it is the single most expensive misunderstanding a KITAS holder can carry.

Cigna Global, AXA, Allianz Care and Bupa Global also write annual expat cover for Indonesia. They have no partnership with this site: we name them without price and without link, because we cannot verify their figures.

BIMC Hospital (Kuta and Nusa Dua) and Siloam Hospitals Bali have international patient departments and deal with international insurers routinely.

🏙️ Jakarta

Siloam and Mayapada run the largest private networks, with cardiac and stroke capability.

💳 Deposit or guarantee

A private hospital asks for a deposit or an insurer’s guarantee of payment before a non-emergency admission. Genki can issue a guarantee where the hospital accepts it; on SafetyWing you usually pay and claim, except on large admissions where the assistance line can arrange one.

✈️ When a transfer is needed

BEFORE YOU COMMIT

Before you commit to a plan

  • Establish what you already have. BPJS through an employer, an employer plan, or nothing. The gap is what you are buying, not the whole.
  • Do not buy a travel plan as annual cover. It is the mistake this page previously encouraged. A 1-to-12-month plan is a bridge.
  • Declare every condition. Underwriting on annual cover produces an exclusion, a moratorium or a loading. An undeclared condition voids the policy.
  • Check the evacuation benefit before the medical limit. In Indonesia, the transfer is the expensive part.
  • Ask about the day the job ends. Employer cover usually ends with the contract, and a gap in cover is what makes a condition « pre-existing » for the next insurer.
  • Buy while the entry age is open. IMG Global Medical accepts applications to 74; lifetime cover requires joining before 65.

My employer provides insurance. Is that enough?

What happens to my cover if I change jobs?

Most employer cover ends with the contract. The practical risk is not the gap itself but what it does to the next application: a condition treated during an uninsured period becomes pre-existing for the new insurer. Arrange the next policy before the current one lapses, even if that means a short travel plan as a bridge.

Does a KITAS give me access to private hospitals?

The permit does not; your cover does. BPJS routes you through an assigned first-tier facility with referral upwards. BIMC, Siloam and Mayapada international departments bill at their own price list, and ask for a deposit or an insurer’s guarantee of payment before a non-emergency admission.

Are pre-existing conditions covered?

Not on the travel plans — neither SafetyWing nor Genki covers them at any price. On annual expat cover the outcome is an exclusion, a moratorium or a loading on the premium, decided by underwriting and always on quote. Declare everything: an undeclared condition voids the policy when you claim.

What about evacuation to Singapore?

Serious cases in Indonesia are routinely transferred, and Singapore is the usual destination. The assistance team chooses the destination and the means; an air ambulance you book yourself is the classic unpaid claim. Ceilings are in the policy wording. Can I use a travel plan while I wait for annual cover?

Yes, and it is the honest use for one. Genki Traveler runs 1 to 12 months in a single policy and can be bought while you are already in Indonesia, with a 14-day waiting period for emergencies if you had no prior cover. SafetyWing runs in 4-week cycles. Treat either as a bridge, not a destination.

Do I need extra cover if I ride a scooter?

Not a separate policy, but you must meet the conditions. SafetyWing covers riding only with a valid motorcycle licence and a helmet. Genki: motor-vehicle use covered with conditions — check the policy wording. No plan covers an unlicensed rider. The detail is in the scooter guide.
