WHO NEEDS IT

Who Needs Health Insurance in Indonesia?

Indonesia has no public healthcare safety net for foreigners. BPJS Kesehatan, the national health insurance, is reserved for Indonesian citizens and KITAP holders (permanent residents). As a tourist, digital nomad, expat on KITAS, or retiree, you are completely on your own when it comes to medical care. What that means in dollars β€” consultation, emergency room, surgery, evacuation β€” is itemised in our guide to healthcare costs in Indonesia without insurance.

⚠️ The reality: Unlike Thailand (which has BUPA hospitals subsidized through some visa programs) or Vietnam (where some employers contribute to BHXH), Indonesia has zero parallel system for foreigners. One serious accident in Canggu and you could face a large bill β€” or worse, be denied treatment until you can prove you can pay.

You should seriously consider health insurance if you are:

πŸ’»

A digital nomad in Bali

Canggu, Ubud, Seminyak β€” Bali is the digital nomad capital of Southeast Asia, and also where most scooter accidents happen. Monthly subscription plans like SafetyWing or Genki are designed for this lifestyle.

🏝️

An expat with a KITAS

Working remotely or for an Indonesian employer. KITAS doesn’t grant BPJS access by default. Comprehensive annual health insurance (IMG Global, Cigna) is non-negotiable for stability. KITAS holders have their own guide: health insurance for KITAS holders in Indonesia.

🌴

Retired in Bali or Lombok

On the Second Home Visa or KITAS Pensiun. Healthcare costs increase sharply with age and Singapore evacuation becomes more likely. IMG Global accepts new applicants up to 74.

πŸ›΅

Renting scooters in Bali

The #1 reason expats need real health coverage in Indonesia. Scooter accidents are extremely common β€” broken bones, road rash, head injuries β€” and costs run into five figures, set by the hospital and the length of stay. See our Bali scooter insurance guide.

🀿

Doing adventure activities

Surfing Uluwatu, diving Komodo, hiking Rinjani, freediving Nusa Penida. You need a plan that explicitly covers these activities. Standard health insurance often excludes adventure sports β€” read the fine print.

πŸ‘¨β€πŸ‘©β€πŸ‘§

Living long-term as a family

School-age kids, maternity, routine check-ups. Family plans from international insurers offer the best value, and direct billing at BIMC and Kasih Ibu eliminates upfront payment headaches.

Short answer: if you’re spending more than a few weeks in Indonesia, you need real coverage. Travel insurance is a stop-gap; health insurance is the long-term solution.

REAL COSTS

What Healthcare Actually Costs in Indonesia

Indonesian private hospitals are modern but expensive for foreigners. Here are real costs at facilities like BIMC, Siloam and Singapore evacuation hubs in 2026.

One night at BIMC Bali

Standard private room. Add 30–40% for an executive room. Singapore-trained doctors, English-speaking staff.

Scooter accident surgery

Tibia/fibula fracture with internal fixation, anesthesia, 4–6 nights of inpatient care, physiotherapy.

Dengue fever hospitalization

IV fluids, daily blood panel, platelet monitoring, 4–7 nights. Common during rainy season Oct–April.

Emergency appendectomy

Laparoscopic surgery, anesthesia, 2–3 nights, follow-up. Standard procedure at BIMC and Siloam.

Childbirth (uncomplicated)

Vaginal delivery at Kasih Ibu or BIMC. C-section adds a bill set by the hospital, the room class and the length of stay. Most plans require 12-month waiting period.

Priced by aircraft and distance

Air ambulance to Singapore

Trauma cases, cardiac events, complex surgeries. Mount Elizabeth or Raffles in Singapore is the standard destination.

πŸ’‘ Real example: A 38-year-old digital nomad in Canggu had a scooter accident in March 2025 β€” broken collarbone, 5 nights at BIMC, surgery, follow-up. Total bill: a five-figure sum. He had Genki Traveler (from €52/month) β€” the entire bill was covered minus the €50 deductible. The premiums paid over five months were a small fraction of it.

PLAN CATEGORIES

Types of Health Insurance Available in Indonesia

Health insurance options for foreigners in Indonesia fall into three tiers, defined by coverage scope, price and the type of lifestyle they’re built for.

TIER 1

Nomad Subscription

$62.72/4 wk (Essential) Β· from $177.50/mo (Complete)

SafetyWing, Genki Traveler

  • βœ… Monthly subscription, cancel anytime
  • βœ… Buy while already in Indonesia
  • βœ… Adventure sports usually included
  • βœ… Genki: 125cc scooter with conditions β€” check the policy wording
  • ⚠️ Lower limits than full health insurance

Best for: Nomads, slow travelers, 1–12 month stays

TIER 2

Mid-tier International

On quote (medical underwriting)/mo

IMG Global, Cigna Silver tier

  • βœ… $1M–$8M coverage limits
  • βœ… Annual policy, renewable for life
  • βœ… Direct billing at BIMC, Siloam
  • βœ… IMG: accepts up to age 74
  • ⚠️ Annual commitment, harder to cancel mid-year

Best for: Most expats, KITAS holders, retirees, families

TIER 3

Premium Global

On quote (medical underwriting)/mo

Bupa Global, AXA, Cigna Platinum, Allianz Care

  • βœ… Very high to unlimited coverage limits
  • βœ… Worldwide including USA option
  • βœ… Maternity, dental, vision included
  • βœ… Concierge claims service
  • ❌ Expensive, especially over 50

Best for: Executives, high-net-worth families, complex needs

πŸ“Œ Important note: there is no widely-available “local” tier for foreigners in Indonesia comparable to Liberty Vietnam or Pacific Cross Thailand. Indonesian local insurers (Allianz Indonesia, Manulife Indonesia, Asuransi Sinar Mas) typically require a KITAS or KITAP and a local employer sponsor β€” they don’t sell directly to nomads or short-term residents. For most foreigners, the realistic choice is between Tier 1 (subscription) and Tier 2 (international annual).

FIND YOUR PLAN

Compare Health Insurance for Indonesia by Profile

Forget marketing comparison tables. The right plan depends on who you are and how you live in Indonesia. Pick the profile that fits β€” we’ll tell you what we’d actually recommend.

πŸ’» Digital nomad in Bali (1–6 months)

Canggu / Ubud / Seminyak Β· age 25–40 Β· scooter daily Β· remote work

What you actually need: flexible monthly subscription, scooter coverage, decent medical limit ($500K+), trip interruption, no commitment.

Often chosen: SafetyWing Complete (from $177.50/month) if you want broader medical coverage including routine care and mental health. Genki Traveler (from €52/month, age-based) if you ride a 125cc scooter β€” motor vehicle use covered with conditions, check the policy wording.

🏝️ Long-term expat with KITAS

Bali / Jakarta Β· age 30–55 Β· 1+ year residency Β· family or solo

What you actually need: annual policy with stable premiums, $1M+ medical coverage, direct billing at BIMC and Siloam, evacuation to Singapore, outpatient care.

Often chosen: IMG Global (on quote, medical underwriting) for solid mid-tier coverage with comprehensive Indonesia-specific benefits. Cigna Global (Silver or Gold tier) is often cited for premium hospital networks β€” ask us for a comparison via the contact form. Skip the local Indonesian insurers β€” most won’t accept you without an employer sponsor.

🌴 Retiree on Second Home Visa

Sanur / Ubud / Lovina Β· age 55–80 Β· long-term Β· pre-existing conditions likely

What you actually need: high coverage limits ($2M+), age-friendly underwriting, ability to cover stable pre-existing conditions, evacuation to Singapore, direct billing.

Often chosen: IMG Global is the most accessible β€” accepts new applicants up to age 74. Cigna Global Gold/Platinum or Bupa Global for higher coverage and broader pre-existing condition handling (with premium loadings). Premiums for ages 65–74 are quoted individually β€” ask us via the contact form.

πŸ‘¨β€πŸ‘©β€πŸ‘§ Family with school-age kids

Sanur / Ubud / Bukit Β· 2 adults + 1–3 kids Β· long-term Β· routine care critical

What you actually need: family plan with kids covered, maternity benefits if planning more, dental and vision, broad outpatient coverage for sick visits, school-age vaccines.

Often chosen: Cigna Global Family or IMG Global with family add-ons. Some premium options (Bupa Global, AXA) include unlimited outpatient β€” useful for families that visit BIMC or Kasih Ibu several times a year. Family premiums are quoted individually β€” ask us via the contact form.

🀿 Diver, surfer or adventure traveler

Komodo / Raja Ampat / Uluwatu Β· adventure activities Β· short-to-medium term

What you actually need: explicit coverage for diving, surfing, hiking and water sports β€” most standard health plans exclude them.

Often chosen: stack Genki Traveler (from €52/month, age-based β€” adventure sports included) with DAN Europe (annual membership, price on the DAN site, for hyperbaric chamber and dive-specific coverage). See our diving insurance Indonesia guide.

πŸ’» BEST FOR NOMADS

SafetyWing

From $62.72 per 4 weeks (ages 10-39) (Essential) β€” Complete plan recommended for Indonesia from $177.50/month

SafetyWing is the most popular nomad insurance in the Bali community for one simple reason: you can sign up in 5 minutes from a beach cafΓ© in Canggu and cancel any month without penalty. The Essential plan is the entry-level travel medical plan; the Complete plan is what we actually recommend for Indonesia because it adds routine care, mental health coverage, maternity benefits and a substantially higher medical limit ($1.5M vs $250K) β€” important given that a Singapore evacuation alone runs to five figures.

Strengths

  • Monthly subscription β€” cancel anytime, buy while already in Indonesia
  • Complete plan: routine + mental health
  • Trip interruption included
  • Easy online claims portal
  • Max age 69 (still accessible for older nomads)

Watch out for

  • Essential plan caps at $250K β€” too low for Bali
  • Scooter coverage requires valid license
  • Pre-existing conditions limited
  • Reimbursement model (you pay first, claim after) β€” $0 deductible for non-US residents

πŸ“Œ Our take for Indonesia: default recommendation for nomads in Bali. Go with the Complete plan, not Essential β€” the difference in evacuation coverage alone is worth it.

πŸ›΅ BEST FOR SCOOTER RIDERS

Genki Traveler

From €52 (Traveler, varies by age β€” €1M limit)

Genki is a German brand and the insurer is Squarelife that has carved out a unique niche in Bali: it’s the only major nomad-style insurer that covers scooters up to 125cc with conditions β€” check the policy wording. For 90% of Bali tourists and nomads, that’s the single most valuable feature in the entire health insurance market here. The Traveler plan (from €52/month, age-based β€” €1M limit) is the version we recommend for Indonesia β€” it covers adventure sports, evacuation, and everything else a Bali lifestyle requires.

Strengths

  • 125cc scooter with conditions β€” check the policy wording β€” unique
  • €1M medical coverage (Traveler)
  • Adventure sports included
  • Monthly subscription model β€” buy while already in Indonesia
  • Backed by Squarelife (the insurer)

Watch out for

  • Max age 69 (Traveler)
  • EUR pricing (FX rate matters)
  • German support hours
  • €50 deductible per claim (€0 if hospitalised)
  • Smaller direct-billing network than IMG/Cigna

πŸ“Œ Our take for Indonesia: if you ride a scooter in Bali (and you will), this is the most pragmatic option in the market. The motor-vehicle clause β€” covered with conditions, check the wording β€” is the feature that decides the most claims in Bali.

πŸ₯ BEST FOR LONG-TERM EXPATS

IMG Global

Price on quote (age-dependent)

IMG Global is a US-based international insurer that offers the best value for most expats settling in Indonesia long-term. Their flagship Global Medical Insurance plan is built for people who need real annual health insurance β€” not just travel coverage. The standout feature for Indonesia: IMG accepts new applicants up to age 74, making it the most accessible international option for retirees in Bali. Direct billing is available at BIMC Bali and several Siloam locations.

Strengths

  • Up to $8M medical coverage
  • Accepts ages up to 74 β€” best for seniors
  • Pre-existing condition options (with loadings)
  • Worldwide network with direct billing at BIMC and Siloam
  • Strong customer service

Watch out for

  • Annual policy (less flexible than monthly)
  • Smaller network than Cigna at top-tier hospitals
  • Reimbursement claims for outpatient
  • US-based β€” time zone gap with Asia
  • Premium increases sharply after 60

πŸ“Œ Our take for Indonesia: the default choice for expats with KITAS, retirees, and anyone over 50 who needs real long-term coverage. Solid balance of price, coverage and Indonesian hospital access.

PRICING BY AGE

Health Insurance Indonesia Costs by Age

Premiums in Indonesia follow the same pattern as everywhere else: they roughly double every 15 years of age, and they spike sharply after 60. Here’s what to actually expect in 2026, based on a single non-smoker on a mid-tier international plan with $1M+ coverage.

AgeNomad SubscriptionMid-tier InternationalPremium Global

⚠️ Important: these are typical individual rates for non-smokers without major pre-existing conditions, in 2026. Premiums are higher with declared chronic conditions, smoking status, or family history of serious illness. Always lock in coverage before age 60 β€” most insurers won’t accept new applicants over 70 (IMG Global up to 74 is the exception).

DECISION FRAMEWORK

How to Choose the Right Health Insurance for Indonesia

Forget marketing brochures. The right plan comes down to four questions you can answer in 5 minutes:

1

How long are you staying in Indonesia?

Less than 6 months: nomad subscription (SafetyWing or Genki). 6–12 months: still nomad subscription, but consider Genki Traveler with €1,000,000 limit. 1+ year: mid-tier international annual plan (IMG Global is the most accessible).

2

Are you riding a scooter?

If yes (and you’re in Bali, almost certainly yes), Genki Traveler is the only mainstream plan that covers 125cc scooters with conditions β€” check the policy wording. If you have a valid motorcycle license + IDP, more options open up β€” but check the engine size limit on every quote.

3

What’s your age?

Under 50: all options open. 50–64: SafetyWing still works (max 74), Genki Traveler works up to 69, IMG Global is solid. 65–74: IMG Global is the most accessible international option. 75+: renewal-only territory β€” don’t wait until you’re 70 to buy your first plan.

4

Do you need direct billing or routine care?

If you visit BIMC or Siloam several times a year for non-emergency care, prioritize plans with direct billing (Cigna Global, IMG Global, Bupa Global). If you mostly worry about catastrophic events (scooter crash, evacuation), reimbursement-model plans like SafetyWing and Genki are perfectly fine.

Still uncertain after these 4 questions? Send us your situation β€” we’ll respond within 24 hours with a personalized recommendation.

HOSPITAL NETWORK

Best Hospitals in Bali & Indonesia for Foreigners

Indonesian private hospitals in Bali and Jakarta are surprisingly good for routine and intermediate care. For serious trauma, complex surgeries or ICU stays, evacuation to Singapore is the standard escalation. Here’s where to actually go β€” and which hospitals work directly with major insurers.

πŸ₯‡ BIMC Hospital β€” Kuta & Nusa Dua

The reference for foreigners in Bali. Singapore-trained doctors, English-speaking staff, direct billing with most international insurers including IMG Global, Cigna Global, Bupa Global, AXA, Allianz Care. Strong ER, decent ICU, capable of handling most non-trauma cases without evacuation.

Best for: emergency care, surgery, scooter accidents, dengue.

πŸ₯ˆ Siloam Hospitals β€” Bali & nationwide

Indonesia’s largest private hospital network. The Bali (Denpasar) and Jakarta (TB Simatupang, Lippo Village) locations work with international insurers. Slightly lower prices than BIMC, broader specialty coverage. JCI-accredited at multiple sites.

Best for: complex specialist care, oncology, cardiology, long-term expat needs.

πŸ‘Ά Kasih Ibu β€” Denpasar & Saba

Family-focused hospital with a strong reputation for maternity, pediatrics and routine care. Several international insurers offer direct billing. More affordable than BIMC for non-emergency visits.

Best for: families, maternity, pediatric care, routine outpatient.

✈️ Mount Elizabeth & Raffles β€” Singapore

The escalation destination for serious cases. World-class trauma centers, top-tier oncology, advanced cardiac surgery. Most international insurers cover Singapore evacuation explicitly β€” verify your plan limit (the sub-limit is in the wording).

Best for: severe trauma, complex surgery, cancer treatment, advanced ICU.

CRITICAL COVERAGE

Medical Evacuation: Why It’s the Single Most Important Coverage in Indonesia

If we had to pick one feature to verify on every health insurance plan in Indonesia, it would not be the medical limit, the deductible, or even the network. It would be medical evacuation coverage.

Here’s why. Indonesia’s healthcare system, even at top facilities like BIMC and Siloam, has real limits. For severe burns, complex neurosurgery, advanced cardiac procedures, certain types of cancer treatment, or any case requiring an ICU bed for more than a few days, doctors will routinely recommend evacuation to Singapore. The clinical decision is rarely the question β€” the question is who pays for the air ambulance.

A medical evacuation flight from Bali to Singapore (Mount Elizabeth or Raffles Hospital) is a transport bill priced by aircraft, distance and medical crew, depending on equipment needs (ventilator, balloon pump, etc.) and medical staff onboard. The aircraft will not lift off until either payment or a Guarantee of Payment from your insurer is confirmed. If you’re uninsured, family members are sometimes asked to wire funds within hours.

What to look for in a plan

  • Evacuation limit β‰₯ $250,000 ideally β€” anything below $100K is a red flag in Indonesia
  • Evacuation to “the nearest suitable facility” language (vs “home country only” β€” useless from Bali)
  • Repatriation included β€” getting you home after stabilization
  • 24/7 medical assistance hotline with a dedicated number for Asia
  • Air ambulance pre-authorization β€” your insurer arranges it, you don’t pay upfront

Both SafetyWing Complete ($1.5M medical limit, evacuation included) and Genki Traveler (€1,000,000 limit, evacuation included) cover Singapore evacuation appropriately for most cases. IMG Global tiers vary β€” check the specific plan. Premium options like Cigna Global, Bupa Global and AXA generally include evacuation in all tiers, with high or unlimited limits β€” read the benefit table.

PRE-EXISTING CONDITIONS

Health Insurance Indonesia: Pre-Existing Conditions

Pre-existing conditions are the most important β€” and most commonly misunderstood β€” part of a health insurance application in Indonesia. The wrong move here can void your entire policy. Here’s the practical guidance.

What counts as “pre-existing”

A pre-existing condition is any medical condition you’ve been diagnosed with, treated for, taken medication for, or experienced symptoms of before the policy start date. This includes:

  • Chronic conditions (asthma, diabetes, hypertension, thyroid disorders)
  • Mental health diagnoses (depression, anxiety) and ongoing therapy
  • Past surgeries (with related ongoing care)
  • Cancer in remission
  • Heart conditions, even if “stable”
  • Pregnancy already in progress

Understanding waiting periods

Most international plans treat pre-existing conditions in one of three ways: permanent exclusion (the most common β€” the condition is simply not covered, ever), moratorium (covered after a 12–24 month period of being symptom-free and treatment-free), or medical underwriting (the condition is reviewed individually, may be covered with a premium loading). Genki and SafetyWing typically use the moratorium approach. Cigna and IMG Global generally use medical underwriting. Each has tradeoffs β€” the moratorium model is faster to apply for but stricter on existing conditions.

⚠️ The single most important rule

Disclose everything. Non-disclosure of a pre-existing condition voids the entire policy β€” not just the claim related to that condition. Insurers actively cross-check medical records when a major claim is filed. The few hundred dollars saved by hiding a condition is never worth the tens of thousands of dollars in denied claims later. When in doubt, disclose. If you’re unsure how to phrase a condition on a form, ask us before submitting β€” we’ve seen every variant.

USING YOUR PLAN

How to Use Your Insurance & Make Claims in Indonesia

There are two main ways your insurance pays for medical care in Indonesia, and knowing the difference saves a lot of stress when something happens.

🟒 Direct billing (cashless)

Your hospital contacts the insurer directly, receives a Guarantee of Payment, and bills the insurer. You only pay your deductible (if any). Available at BIMC, Siloam, Kasih Ibu with most international insurers like Cigna Global, IMG Global, Bupa Global, AXA Global Healthcare and Allianz Care.

Best for: planned care, hospital admissions, maternity, surgery.

πŸ”΅ Reimbursement model

You pay the hospital upfront (cash or card), then submit receipts and a claim form to your insurer for reimbursement. Standard for SafetyWing, Genki, and most outpatient claims. Reimbursements usually take 2–4 weeks via bank transfer.

Best for: outpatient visits, walk-in clinics, dengue treatment, pharmacy.

5-step claims checklist for Indonesia

  1. Call your insurer’s 24/7 hotline first β€” even before going to the hospital if you can. They’ll tell you which facility to go to and pre-authorize direct billing if available.
  2. Keep all original receipts and discharge documents β€” Indonesian hospitals provide them in English on request. The discharge summary is often required for claims.
  3. Take photos of the medical reports immediately. Originals can get misplaced; cloud-stored photos are accepted by most insurers.
  4. File the claim within the deadline β€” usually 30–90 days from treatment. Late claims are sometimes accepted but not guaranteed.
  5. Follow up if no response in 2 weeks. Insurance bureaucracies can lose paperwork; a polite email reference is usually enough to restart the process.

BPJS VS PRIVATE VS INTERNATIONAL

BPJS Kesehatan, Local Private and International Plans: At a Glance

Three systems coexist in Indonesia and most foreigners only ever use one of them. Here is how they compare on the questions that matter β€” who can join, where you can be treated, whether evacuation is included β€” without prices we have not verified.

CriterionBPJS Kesehatan (national scheme)Indonesian private insurersInternational / nomad plans
Who can joinIndonesian citizens and permanent residents (KITAP); foreign employees on a work KITAS may be enrolled by their employer under the national scheme’s rules β€” the rule and its enforcement change, confirm with your employer. Tourists, nomads and most KITAS holders: no.Mostly residents with a KITAS/KITAP and often an employer sponsor; rarely sold directly to short-term foreigners.Any nationality, bought online, often while already in Indonesia (SafetyWing, Genki); IMG Global with medical underwriting.
Where you are treatedPublic hospitals and puskesmas, referral chain from primary care; long waits, limited English.Indonesian private networks (Siloam, Mayapada, some BIMC access) β€” check the hospital list.Any hospital; direct billing where the insurer has an agreement (BIMC, Siloam, Kasih Ibu for the international annual plans; reimbursement model on SafetyWing and Genki).
Evacuation to SingaporeNo.Usually no, or domestic only β€” read the benefit table.Evacuation benefit is standard on the plans named here; the sub-limit is in each wording.
Coverage outside IndonesiaNo.Rarely.Worldwide (SafetyWing: USA optional; Genki: USA/Canada chosen at sign-up; IMG: with or without USA).
Pre-existing conditionsCovered within the public system for members.Underwriting and waiting periods, insurer by insurer.SafetyWing and Genki: not covered; IMG Global: medical underwriting, acceptance not guaranteed.
PriceSet by regulation, in rupiah, by class β€” not in our verified data.On quote.SafetyWing Essential $62.72 per 4 weeks (10-39), Complete from $177.50/month; Genki from €52/month (age-based); IMG Global on quote.

Our reading: do not build a plan around BPJS. Even where enrolment is possible for a working expat, it is a public-hospital system with a referral chain; keep an international plan for the evacuation and the private-hospital access that foreigners actually use in Bali and Jakarta.

WHERE YOU LIVE MATTERS

Hospital Quality by Region: Jakarta and Bali Are Not Indonesia

The same policy buys very different care depending on where you use it. This is the part of the decision that price tables never show.

Jakarta and Greater Jakarta

The deepest private network in the country: Siloam (several sites), Mayapada, Pondok Indah, JCI-accredited facilities and the specialists that Bali refers to. Most complex care that does not go to Singapore happens here. Direct billing with the international annual plans is routine.

Bali (south and Ubud)

Good for emergencies, surgery, maternity and routine care at BIMC, Siloam Denpasar and Kasih Ibu; intensive care and complex surgery still escalate to Jakarta or Singapore. North and east Bali are an hour or two from these hospitals β€” relevant if you live in Amed, Lovina or Munduk.

Lombok, the Gilis, Yogyakarta, Surabaya, Medan, Makassar

Private hospitals exist (Siloam in several of these cities) and handle intermediate care; the international insurers’ direct-billing lists are shorter and the Gilis are a boat ride from anything. Expect reimbursement rather than cashless treatment, and plan the transfer to Bali or Jakarta for serious cases.

Komodo, Flores, Sumba, Raja Ampat, the Mentawai, most of Sumatra and Kalimantan

Clinics that stabilise. Everything else is an evacuation β€” by boat, then by air β€” organised by the insurer’s assistance line. This is where the evacuation clause, not the medical limit, is the whole policy. Divers and surfers living or working here should read our diving insurance and evacuation to Singapore guides.

APPLYING

How to Apply for Health Insurance in Indonesia: 5 Steps

  1. Fix the horizon. Under a year and flexible: a monthly plan (SafetyWing Essential renews every 4 weeks; Genki Traveler runs 1 to 12 months). A year or more, a KITAS, a family: an annual international plan (IMG Global with underwriting; Cigna, AXA, Bupa, Allianz on quote via the contact form).
  2. Declare, do not curate. Every diagnosed condition, every daily medication. SafetyWing and Genki exclude pre-existing conditions outright; IMG Global underwrites the file and charges the card only after underwriting. Non-disclosure voids a policy, not just a claim.
  3. Choose the area of cover. Worldwide minus USA is the usual expat choice (SafetyWing: USA optional; Genki: USA/Canada at sign-up; IMG: with or without USA). Remember home-country limits: SafetyWing Essential 30 days per 90 (15 for US residents), Genki six weeks of emergencies per stay.
  4. Read four lines before paying. The evacuation benefit and its sub-limit; the motorbike clause (SafetyWing: licence and helmet; Genki: motor vehicle use with conditions β€” check the policy wording); the adventure-sports treatment (SafetyWing: paid add-on on Essential, included in Complete, $100,000 lifetime cap); and the deductible ($0 on SafetyWing for non-US residents; €50 on Genki, €0 if hospitalised).
  5. Set up the day-one kit. Policy number and the 24/7 assistance number saved offline, the direct-billing hospital list if you have one (BIMC, Siloam, Kasih Ibu for the international annual plans), and the claim portal login. Then send us the wording if a clause is unclear β€” free, no obligation.

FAQ

Health Insurance Indonesia: Frequently Asked Questions

Is health insurance mandatory in Indonesia?

No. Indonesia does not require health insurance for any visa category, including KITAS, KITAP, the Second Home Visa or the Investor Visa. There’s no border check for proof of insurance. But foreigners cannot access the public BPJS system, so private coverage is effectively a necessity for anyone planning to live or stay long-term.

Can foreigners use BPJS in Indonesia?

Effectively no. BPJS Kesehatan is reserved for Indonesian citizens and KITAP (permanent resident) holders. KITAS holders are generally not eligible unless enrolled through an employer under the national scheme’s rules. Even if you qualified, BPJS is mainly used at public hospitals where waiting times, language barriers and equipment levels make it impractical for foreigners. Stick with private international plans.

How much does expat health insurance cost in Indonesia?

Expect $62.72 per 4 weeks on SafetyWing Essential (ages 10-39) or from €52 per month on Genki Traveler; annual international plans are on quote and depend on your age, coverage tier and pre-existing conditions. Nomad subscription plans (SafetyWing, Genki) start at $62.72 per 4 weeks (ages 10-39) for under-40s (SafetyWing Essential). Mid-tier international plans (IMG Global, Cigna Silver) and premium global plans (Bupa Global, AXA, Cigna Platinum) are priced on quote and rise sharply with age.

What’s the best health insurance for expats in Bali?

It depends on your situation. SafetyWing Complete (from $177.50/month) for nomads who want flexibility. Genki Traveler (from €52/month, age-based) if you ride a 125cc scooter with conditions β€” check the policy wording. IMG Global (on quote, medical underwriting) for stable long-term expats with KITAS. Cigna Global (Silver/Gold) for premium hospital networks. Skip Indonesian local insurers β€” they typically require an Indonesian employer sponsor.

Can I get health insurance in Indonesia over 60?

Yes, but options narrow significantly. IMG Global accepts new applicants up to age 74 β€” the most accessible international option. Cigna Global typically accepts applications up to 64 (renewable for life). Premium options like Bupa Global have higher age caps but with substantial premium loadings. Premiums for ages 65–74 are quoted individually β€” ask us via the contact form. Lock in coverage before 60 if possible.

Does travel insurance work for long-term stays in Indonesia?

Not really, beyond 3–6 months. Travel insurance is designed for trips, not residency. Most policies cap at 30–90 days and don’t cover routine care, regular check-ups, or chronic conditions. If you’re staying in Indonesia 6+ months, transition to a proper nomad subscription (SafetyWing/Genki) or an annual health insurance plan.

How does direct billing work at Indonesian hospitals?

When you arrive at the hospital, you present your insurance card. The hospital contacts your insurer, receives a Guarantee of Payment, and bills them directly. You pay only your deductible (if applicable). Available at BIMC, Siloam and Kasih Ibu with major international insurers including Cigna Global, IMG Global, Bupa Global, AXA Global Healthcare and Allianz Care.

Are pre-existing conditions covered?

It depends on the insurer. Minor stable conditions may be covered after a moratorium (typically 12–24 months symptom-free) or with a premium loading. Major conditions (cancer, heart disease, autoimmune) are usually permanently excluded on new policies. Always disclose everything β€” non-disclosure voids the entire policy, not just the related claim.

What’s NOT covered by health insurance in Indonesia?

Common exclusions include cosmetic procedures, dental care (unless added), vision (unless added), fertility treatments, self-inflicted injuries, scuba diving (without specific add-on), pre-existing conditions (varies), experimental treatments and injuries from illegal activities β€” including drug use. Always read the exclusions list before buying.

Do I have to join BPJS if I work in Indonesia on a KITAS?

Foreign employees on a work KITAS may be enrolled in BPJS Kesehatan by their employer under the national scheme’s rules; the rule and its enforcement have changed several times, so confirm with your employer and immigration rather than with a forum. Either way, BPJS is a public-hospital system with a referral chain: keep an international plan for private hospitals and evacuation.

Can I use international health insurance at Indonesian hospitals?

Yes. BIMC, Siloam and Kasih Ibu bill international annual insurers directly once a Guarantee of Payment is issued; SafetyWing and Genki work mostly on reimbursement (you pay, then claim through the dashboard or app), with direct billing where the hospital accepts it. Carry the policy number and the 24/7 assistance number β€” the hospital calls the insurer, not the other way round.

Which health insurance do I need for a digital nomad visa or a long stay in Bali?

No Indonesian visa category requires proof of health insurance at entry. For a stay of weeks to months, a monthly plan (SafetyWing Essential at $62.72 per 4 weeks for ages 10-39; Genki Traveler from €52 per month, age-based) is what most nomads in Canggu and Ubud use; for a year or more, or with a family, an annual international plan such as IMG Global (medical underwriting, sign-up until 74) is the usual step up. Our digital nomad insurance Indonesia guide compares the two routes.

How we rank providers

Providers are ranked on Indonesia-specific coverage quality, not on commission rates. Our scoring weights direct-billing access at BIMC, Siloam and Kasih Ibu, scooter coverage terms (license requirements and engine size limits β€” central to Bali life), medical evacuation limits to Singapore, age acceptance and renewal guarantees, pre-existing condition handling, and how well the geographic scope matches a typical Indonesia expat lifestyle. We do not accept payment for placement and we publish weaknesses alongside strengths for every plan we cover. Read the full affiliate disclosure.
